Enhanced Care Management (ECM)
Extra support for children and families when life feels overwhelming.
At Child Guidance Center, we understand that families are often balancing emotional, behavioral, medical, school and everyday life challenges all at once. Enhanced Care Management (ECM) helps bring services and support together by connecting families to resources, coordinating care, and providing guidance every step of the way - so families do not have to navigate it alone.
ECM is a free Medi-Cal benefit for CalOptima members that provides hands-on, personalized support for children and families with complex needs.
Who ECM Supports?
ECM may be right for children and families who:
Have mental health or behavioral challenges
Are managing complex medical or developmental needs
Are experiencing housing instability or major life stressors
Are involved in multiple systems (healthcare, school, foster care, etc.)
Need extra support staying connected to care and services
What ECM Provides
Whole-person, family-centered care—looks at all factors impacting your child and family
In-person, relationship-based support—not just referrals or phone calls
Ongoing partnership and care coordination to help promote stability, connection, and progress
No Cost Services provided for eligible CalOptima members with Medi-Cal
How We Support Your Family
A Dedicated Lead Care Manager who builds a trusted, supportive relationship with your family
Flexible Support that meets you where you are - at home via telehealth, school, any of CGC 6 clinics, or in the community
Assistance Accessing Care—medical, mental health, developmental and community-based services
Case Coordination with providers, schools, and other support systems to ensure everyone is working together
Connection to Resources such as housing support, food assistance, transportation, and employment services
Additional Support during major transitions, hospital visits, crises or other challenging life events
Ongoing Advocacy and Guidance for your child and family every step of the way

If eligible, your family will be connected with a dedicated Care Manager who will provide ongoing support, coordination, and guidance every step of the way.
